The Center for Health & Sports Medicine (CHSM) is a modern primary care and sports medicine practice dedicated to proactive, personalized healthcare. With locations serving the greater Jacksonville and St. Johns communities, CHSM combines family medicine, sports medicine, wellness, recovery, and performance services under one roof to help patients feel, move, and live better.
